A starship or facility has numerous personnel aboard, and thus the Chief Medical Officer cannot be expect to do all the work required. The Asst. Chief Medical Officer assists the Chief in all areas, such as administration and application of medical care.

In addition to these duties, the Assistant Chief Medical Officer (ACMO) also has the responsibility to take over for the Chief Medical Officer should it be required. Typically, the ACMO is a Lieutenant Junior Grade on most ships and might hold the rank of Lieutenant or higher on large ships and bases. They are required to meet the same educational requirements as a Chief Medical Officer but usually don't have the experience or some of the other advanced degrees a Chief might have.
